Meadow Wood Rehab apartments are located on the Good Shepherd Services Campus in Seymour. The contemporary, comfortable apartments offer a safe alternative for those that need assistance after surgery or illness. Our rehab apartments are fully furnished allowing a worry free move-in. Each apartment offers a living room, dining room, kitchen and a large bathroom with roll in shower. Rehab services are provided either in our rehab clinic or in your apartment, which ever meets your needs and goals.
Flat Fee Daily Rate to Include:
Nursing care and management by licensed nurseAssistance with activities of daily living by trained caregivers 24 hours daily Medication administration (medications are not included)
Walker and/or wheelchair

Not Included in Daily Rate:
Access to Occupational, Physical or Speech therapy on outpatient basisAccess to physician or nurse practitioner (Prevea Clinic) on campus
Transportation to medical appointments
Medications, DME other than wheelchair or walker
